THE ORGANIZATION: As a non-profit organization Conard House, Inc. builds welcoming communities and caring relationships that empower people and restore hope. The position is with Conard House Supportive Housing Program, which provides on-site supportive services to formerly homeless adults with behavioral and medical health needs. Services include: income advocacy, money management, supportive case management and referrals to health care services. This position is included in the Collective Bargaining Agreement with SEIU 1021. QUALIFICATIONS: Associate’s Degree and higher, or equivalent experience working with adults with mental health, substance use, or other life challenges. Demonstrated case management skills and knowledge of entitlement, community resources and housing programs. Must have good written communication skills. Experience with case management software, Panoramic a plus. Working knowledge of HIPAA regulations and mandated reporting requirements. Ability to work as a team member towards common goals and objectives. Must successfully complete a background check after conditional job offer.

DUTIES:

- Maintain accurate records in the Panoramic and Truelink systems to meet compliance requirements

- Provide ongoing case management and connect clients with community resources (e.g., medical, psychiatric, substance use, vocational)

- Participate in staff meetings, case conferences, and training sessions

- Provide representative payee services and create financial disbursement plans for clients

- Manage client budgets, ensuring bills and essential needs are covered

- Maintain up-to-date financial records in the Panoramic system

- Help clients with Social Security appeals, overpayment waivers, and utility discounts

- Monitor client accounts for unusual activity and notify relevant agencies of entitlement changes

- Submit accurate monthly reports and timesheets by required deadlines

- Report emergencies, work-related injuries, or breaches of privacy immediately

- Maintain professional boundaries with clients and collaborate with internal and external stakeholders

- Notify Program Director of absences or schedule changes in advance

- Provide services to residents in a manner that is supportive, constructive and non-judgmental.

- Maintain up-to-date case files in adherence with agency and funding department documentation standards.

- Respond in crisis situations and provide de-escalation and conflict resolution assistance.

- Perform general office duties such as phones, filing, etc.

- Attend on-site and agency meetings and trainings as scheduled.

- Perform other duties as assigned
