Conduct initial assessments and reassessments, including but not limited to chemical nicotine dependence instruments to evaluate physical and mental health behaviors. In collaboration with a case manager and client, develop service plans to identify mutually agreed upon long and short-term recovery-related goals.

Evaluate clients’ needs based on the substance use harm reduction model and offer harm reduction or coping techniques to reduce clients’ risky or highly misused behaviors.

Connect clients to outpatient behavioral health and substance abuse outpatient programs; assist in accessing inpatient substance abuse rehabilitation; assist with adherence to medication regimens; activities for daily living; nutritional services; recreation program; and other necessary services to clients to increase their capacity for independent living. Also, monitor and ensure that clients are linked to needed services.

Facilitate individual and group sessions, including harm reduction, double trouble, and recovery model groups. Educate clients on treatment options and offer ongoing psychoeducation as specified in the service plan(s). Assist the client in identifying behavioral triggers that interfere with their substance-using treatment and recovery.

Education and Experience Requirement(s)
High School Diploma or Associates degree and Credentialed Alcohol and Substance Abuse Counselor (CASAC) with five (5) years of substance abuse counseling experience; Bachelor’s degree with CASAC-T and 2 years of experience. Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals, and their families a plus. Bilingual - Spanish, a plus.
Certifications & Licenses Required
Valid New York State CASAC or CASAC-T Certificate.
Salary : $55,000 - $63,000
