Comunilife Inc. is looking for experienced Case Managers for its new Bronx program, El Hogar. The Case Manager is responsible for assessing the resident's needs and developing a case plan to ensure that the resident is linked to needed services both inside the residence and in the community.  Assist residents in developing necessary skills to live more independently in permanent housing.  Responsible for general case management, working with residents’ families and significant others for the residence, which provides congregate treatment services, i.e., case management and recreational activities to the frail elderly and senior residents of the El Hogar program. Other responsibilities include working harmoniously with the El Hogar Program staff in accordance with Comunilife principles and guidelines.  These full time positions are eligible for Comunilife's generous benefits and paid time off package. This position is salaried upto $55,000 a year.
 
Essential Duties:
 
  • Ensure that each client is receiving appropriate and effective case management services           
  • Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed-upon long and short-term goals.
  • Follow DOH procedures regarding the referral process and the completion of forms and statistical records.
  • Conduct face-to-face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.
  • Conduct monthly apartment inspections.
  • Provide assistance in accessing primary medical care; assistance with adherence to medication regimens; teach activities for daily living; nutritional services; recreation programs; services; prevention with positives; and other necessary services to clients to increase their capacity for independent living. 
  • Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs.
  • Provide crisis intervention, coordination, and follow-up for residents experiencing behavioral or medical challenges.
  • Promote health, wellness, and life skills through individual sessions and group workshops on topics such as nutrition, budgeting, community engagement, and recreation.
  • Assist with move-ins and orientation to housing, including education on tenant rights and responsibilities.
  • Assist residents in accessing medical and behavioral health care, medication management and home health aide services.
  • Ensure that clients pay the balance of their rent, telephone and utility bills.
  • Support and assist residents with entitlements, benefits, and financial stability (e.g, SSI, Medicaid, SNAP).
  • Provide a monthly caseload update to the Program Director.  Document all direct activities, including resident served case closures and direct service advocacy interventions.
  • Monitor housing stability, coordinate with Property Management for lease renewals, and support eviction prevention efforts.
  • Provide and coordinate all supportive services as needed.
  • When necessary and appropriate, accompany residents in medical appointments and/or arrange ambulation services.
  • Collaborate with community partners to ensure residents’ needs are met across service domains.
  • Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living.
  • Maintain accurate case notes, service plans, and documentation in accordance with DOH ESSHI standards and program policies.
  • Complete quarterly reporting per DOH ESSHI.
  • Participate in weekly supervision.
  • Submit a weekly schedule of visits.
  • Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors.
  • Establish a late day schedule, i.e., 11:00 a.m. to 8:00 p.m., or 10 a.m. to 7:00 p.m. for one (1) day per week to ensure that all residents receive case management services.
Qualifications:
Education and Experience Requirement(s)
Bachelor's Degree in Human services preferred. An Associates degree with at  least 4 years of human services experience; Knowledge of ESSHI or supportive housing programs; Direct service experience working with seniors and/or individuals with Medical, substance abuse, serious mental illness, and homelessness needs; 5 Day/35 work week Position (In-Office/Some Field Work); Excellent oral and written communication skills; Proficient in Google Workspace program/Microsoft Office Suite
