Certification: Basic Life Support

The ideal candidate will have strong clinical skills, attention to detail, and the ability to deliver high-quality care in a home health setting. This role requires experience with Epic Homecare, strong communication abilities, and a proactive approach to patient care.
Key Responsibilities
- Evaluate and monitor patients’ progress in the home setting
- Identify and respond to sudden or subtle changes in a patient’s medical condition
- Communicate updates clearly to physicians, patients, and family members
- Begin treatment and monitor medication dosages
- Respond to medical emergencies and notify appropriate healthcare providers
- Complete all necessary documentation prior to patient transfers
- Maintain up-to-date and accurate patient records
- Develop and implement effective, individualized care plans
- Education: Bachelor of Science in Nursing (BSN)
- Experience: Minimum 2 years of LPN experience, specifically in a home health setting
- Technical Skills: Experience with Epic Homecare system
- Licensure: Active RN License (Note: RN license mentioned in requirements may need clarification if position is for an LPN role)
- Basic Life Support (BLS) – Required
- COVID-19 vaccination – Required
- Strong interpersonal and communication skills
- Ability to work independently in a home-based care environment
