This is a non-bedside role for a Licensed Practical Nurse (LPN) to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.

This is a non-bedside role for a Licensed Practical Nurse (LPN) to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
Your Contribution
- Create, update, and review patient care plans based on the provider’s plan of care
- Conduct monthly telephonic check-ins with patients to assess health status and medication adherence
- Review RPM synced vital signs and escalate concerns to the provider when appropriate
- Coordinate follow-ups, scheduling, and documentation updates
- Ensure all documentation meets Medicare CCM and RPM compliance guidelines
- Accurately document all patient interactions within multiple EHR systems
- Active LPN License (Multistate Nurse Compact License Required)
- Strong verbal and written communication skills
- Minimum 1 year in related Chronic Care Management services
- Experience working with EHR systems
- Ability to work independently with minimal supervision
- Reliable high-speed internet and a quiet, HIPAA-compliant workspace
