What are we looking for in a Case Manager?
In this position, Case Managers provide intensive community support to individuals who have a diagnosed behavioral health challenge, co-occurring substance use disorder, and are frequently utilizing emergency services or having frequent contact with the criminal justice system. The Case Manager supports clients who are participating in one of Yakima County’s therapeutic courts, such as drug court or mental health court, to engage the client prior to their release from jail and ensure that they are participating in the required treatment. The Case Manager will coordinate with the courts to provide updates on each client’s engagement with treatment, and to offer consultation and recommendations to improve the client’s outcomes. Frequent coordination with law enforcement, emergency medical services, corrections departments, and local service providers is necessary in this role. This position requires a confident clinician who is comfortable working outside of the office in the community and within corrections settings, has exceptional communication skills, and enjoys collaboration with multiple partner organizations. The work schedule is Monday-Friday 8am-5pm.
Case Manager duties may include:
- providing intensive support and case management services to high-risk clients in local jails and emergency services
- coordinating with clients’ treatment teams and community providers to develop treatment and discharge plans, including planning for treatment upon release from correctional or medical facilities and general re-entry planning
- participating in staffings for each client being served
- providing updates to the therapeutic court about client participation in treatment and making recommendations to improve outcomes
- assessing clients for safety and risk to self or others and taking appropriate crisis intervention steps when indicated
- reporting on contact and outcome data, including compliance reporting
- completing all documentation in a respectful, client-centered manner
Qualifications:
- Bachelor's degree in social work, psychology, or related field
- Must be eligible for registration as Agency Affiliated Counselor
- Must have valid driver's license and vehicle liability insurance
