Job Summary:
Provides personal care and related services in the home. Functions under direct instruction and supervision of an RN or therapist.
Essential Functions:
Follows written instructions for patient care as prepared by an RN or Therapist.
Assists patients with personal care (which includes lifting and transferring patients without assistance) to preserve good personal hygiene while maintaining a healthful, safe environment.
Performs simple procedures as an extension of therapy services, such as ambulation, ROM, and exercise.
Reports changes in patient's condition and needs to Supervisor or RN.
Maintains record of services performed and of apparent condition of patient.
Establishes and maintains a good working relationship with the patient/family and co-workers of the Hospice.
Participates in patient care conferences to provide input requiring continuum of patient care.
Performs household services essential to health care at home.
Additional Responsibilities:
Performs other related duties as assigned or requested.
Conforms to all applicable Hospice policies and procedures.
Participates actively in continuing education and in-services.
Maintains confidentiality of patient information and business trade practices.
Assumes accountability for reporting incidents and complaints according to Hospice policy.

Participates in on-call rotation
Reliable transportation.
Minimum Position Qualifications:
· Education: High school diploma or GED
· Experience: 1-year hospice aide experience preferred
· Training: Successful completion of training and competency evaluation program or competency evaluation program or nurse aide training and competency evaluation program approved by the state without a contiguous 24 months lapse
· License / Certification: Driver’s license and proof of current auto liability insurance; No Listing in the OIG Excluded Provider Listing; listed in good standing on state nurse aide registry or requirements of a state licensure program meeting federal regulations
