JOB SUMMARY: 

The Disabilities Coordinator is responsible for the coordination, provision, and referral of all aspects of service to those children who have been identified as having a disability. The disabilities coordinator is responsible for the tracking and implementation of special services. This person must ensure that families are supported during the process of obtaining special services or once a child receives services, that the services are being implemented. This position is responsible for ensuring the program meets all rules, regulations, and mandates for disability services. Plan and provide training related to disability services available to families and supports for staff, and policies and procedures for the department and agency. Ensure monitoring, agency collaboration and tracking of disability services.

Requirements

REQUIRED QUALIFICATIONS:

  1. Bachelor’s degree in early childhood, Human Development, related fields, or experience.   
  2. Bilingual in Spanish and English, preferred. 
  3. Ability to work with infants, toddlers, preschoolers, prenatal women, and their families.
  4. Valid driver’s license and proof of liability insurance.
  5. Provide/maintain all required immunizations and/or vaccinations.
  6. Complete all required background checks.

WORK SCHEDULE DEMANDS:

  • Full-time/full-year, 35 hours per week position. 
  • May need to accommodate potential open or close of buildings as needed. 
  • Due to the nature of the job, this individual must be flexible with their schedule to accommodate the client population. 
  • Occasional evening and weekend hours for community events. 
  • May be required to work at any CCAP location. 


KEY RESPONSIBILITIES:

  • Orient parents to their roles and responsibilities as an advocate of their child.
  • Utilize a system that tracks screenings, assessments, referrals, and evaluation.  
  • Inform parents of parent training’s, workshops, etc. within the agency and outside of agency related to advocacy and special education services.
  • Act as the liaison between the family and LEA to coordinate meetings and services.
  • Provide monthly tracking data to director and Policy Council.  
  • Conduct classroom observations.  
  • Support recruitment efforts for all early childhood programs. This may include attending community events.
  • Join other Disability HS specialists in the state for monthly meetings.
  • Oversee itinerant schedule and provision of service. 
  • Utilize Child Plus as a tool to track correspondence with families, track progress related to referrals and monitor those files for completion and compliance related to special services. 
  • Coordinate outreach screening services with LEA and ensure all students are screened in 45 days.   
  • Support management team with community assessment, PIR, annual report data.  
  • Attend all required staff and management meetings.
  • Other duties as assigned.
