Description:The Supportive Housing Coordinator provides direct client support and coordinates administrative functions for the Supportive Housing services while ensuring accurate documentation, program compliance, and efficient daily operations.
Key Duties:
· Provide direct support to clients by conducting intake interviews, gathering required information, and completing program documentation, including income verification and rent calculations.
· Establish, maintain, and update accurate and confidential client files in accordance with agency policies and regulatory requirements.
· Ensure compliance with all applicable U.S. Department of Housing and Urban Development (HUD), Illinois State, St. Clair County, and agency policies, procedures, and program regulations.
· Collaborate with the Housing Resource Center of St. Clair County, the Intergovernmental Grants Department and other community partners to coordinate referrals and secure appropriate housing resources for clients.
· Collect, verify, and accurately document client information in the Homeless Management Information System (HMS) and other required database and reporting systems.
· Prepare, review, and submit required reports to funding agencies and external partners by established deadlines.
· Monitor client eligibility and program participation to ensure continued compliance with funding and program requirements.
· Participate in community outreach, networking, and engagement activities to promote supportive housing services and strengthen partnerships withing the service area.
· Communicate effectively with clients, community partners, and internal staff to coordinate services and facilitate successful housing outcomes.
Reports to: Program Manager of Supportive Housing
Education and Certifications:
· Bachelor’s degree in social work, Psychology, Human Services, Sociology, Counseling, or a closely related field that provides knowledge of human behavior, case management, and social service delivery required.
· Valid driver's license with a clean driving record required.
· Must maintain current automobile insurance that meets Illinois’ minimum coverage requirements.
· Must be able to travel throughout the St. Clair County and surrounding service areas to meet with clients, community partners and attend outreach activities.
Compensation: $22.00 to $23.00 per hour
Pay: $22.00 - $23.00 per hour
