Position Overview
Compounding Pharmacy Solutions is seeking a highly motivated Clinical Home Infusion Pharmacist to join our growing team. This position offers an exciting opportunity to play a key role in managing home infusion therapies, sterile compounding operations, clinical monitoring, and patient care coordination.
The ideal candidate will have strong clinical knowledge, excellent communication skills, and experience in sterile compounding, hospital pharmacy, home infusion, or ambulatory infusion services.
Pay is dependent upon experience.
Yearly bonuses paid.
Responsibilities
- Verify physician orders for accuracy, appropriateness, and safety.
- Review patient medical histories, allergies, laboratory results, and medication profiles.
- Dispense sterile compounded infusion medications in compliance with state, and federal regulations.
- Conduct patient assessments and provide clinical recommendations when appropriate.
- Monitor therapy effectiveness and identify potential medication-related problems.
- Collaborate with physicians, nurses, home health agencies, and other healthcare professionals to optimize patient outcomes.
- Provide medication counseling and education to patients and caregivers.
- Maintain accurate and compliant documentation within EMR/EHR systems.
- Participate in quality assurance initiatives and regulatory compliance activities.
- Support pharmacy operations and workflow to ensure timely patient care.
- Participate in rotating on-call coverage for infusion patients as needed.
Required Qualifications
- Required Qualifications
- Active Pharmacist License in the State of Texas.
- Doctor of Pharmacy (PharmD) or equivalent pharmacy degree.
- Strong knowledge of pharmacology, medication therapy management, and infusion services.
- Excellent communication, clinical, and organizational skills.
- Ability to work effectively in a fast-paced healthcare environment.
Preferred Qualifications
- Experience in home infusion, sterile compounding, hospital pharmacy, critical care, or ambulatory infusion settings.
- Knowledge of USP <797> requirements.
- Familiarity with managed care, specialty pharmacy, and infusion reimbursement processes.
- Experience with CareTend, CPR , or similar pharmacy software.
- Previous on-call or after-hours pharmacy support experience.
