Job Title: Nurse Practitioner
We are seeking a compassionate and dedicated Nurse Practitioner to join our team at ComplexCare Solutions (CCS). As a CCS Nurse Practitioner, you will play a vital role in delivering high-quality healthcare services to our members nationwide.
About Us
CCS specializes in providing in-home and virtual assessments for health plans. Our mission is to improve clinical outcomes by evaluating current health status, identifying gaps in care, and developing personalized care management plans.
Responsibilities
- Perform comprehensive annual health assessments of high-risk health plan members to identify medical conditions, prior and current medical history, and conduct medication reviews.
- Provide patient education on health conditions, medications, and nutrition, emphasizing the importance of consistent monitoring with their Primary Care Physician.
- Identify opportunities for preventive care, health promotion, home safety, and illness prevention.
- Complete documentation within 24 hours from each visit.
- Communicate urgent medical needs, including prescription requirements, with the member's Primary Care Physician.
- Document clinical escalations and communicate with managers as per CCS policy.
- Perform, document, and communicate results of in-home Point of Care (POC) Testing when directed.
- Maintain ongoing Continuing Education as mandated by state licensure and adhere to professional standards.
Qualifications
- Active, unencumbered nursing license.
- Nurse Practitioner Certification from ANCC or AANP.
- Board Certified through AANP or ANCC.
- Current CPR certification.
- Compliance with recommended vaccination and testing requirements.
- Home Health experience a plus.
- Ability to effectively communicate with elderly and chronically ill patients and families.
- Understanding of Medicare, Medicaid, and Health Plan benefit structures beneficial.
- Excellent customer service skills.
- Bilingual or multilingual a plus.
