Essential Functions
The Home Health Aide is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional wellbeing, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures.
- Understand and adhere to established agency policies and procedures
- Perform personal hygiene and hands-on care as assigned
- Complete appropriate visit records in a timely manner as per agency policy
- Reports changes in the patient’s condition and needs to the RN
- Performs household services essential to health care in the home environment
- Ambulates and exercises the patient as needed
- Perform simple procedures as an extension of the therapy or nursing services
- Assists with medications that are ordinarily self-administered as needed
- Attends in-service and continuing education programs as scheduled as necessary
Job Qualifications and Requirements
- Must meet Medicare Conditions of Participation for Home Health Aide training program and competency
- Have a sympathetic attitude toward the care of the sick and elderly
- Ability to carry out directions with exceptional communications skills
- The maturity and ability to deal effectively with the demands of a home care position
License Requirements
- Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
