Schedule: Tues 7:45am-5:30pm, Thurs & Fri 8am-4:30pm or 8:30am-5pm- possibility to leave at 1pm
Temp Pay: $50/hour

Position Summary: The Maternal Child Health Nurse Care Coordinator serves as an integral member of the healthcare team and is responsible for coordinating the care of OB patients from initial entry into care to postpartum visit, with priority on the subset of high-risk OB patients, to ensure the optimum health of these increased risk patients. The role promotes continuity of care for OB patients and newborns, and serves as a resource to clinicians and other clinical staff involved in prenatal care. The role will focus on facilitating timely entry to prenatal care, and identifying and coordinating appropriate delivery of prenatal and newborn care as provided by the healthcare team, at the direction of the prenatal clinician. The role supports the scheduling and delivery of appropriate health care and community services during the pregnancy and post-partum periods, and initial well child visits for the newborn. This position reports to the Care Coordinator Nurse Manager (CCNM), and works closely and collaboratively with Clinical Care Nurse Coordinators (CCNCs), medical leadership, and nurses conducting prenatal intakes at all sites.

Job Responsibilities and Performance Standards:

  • Coordinates with prenatal clinician and care team to develop a prenatal care plan addressing all identified problems, and support care team in carrying out, and updating, the care plan in a timely manner.
  • Coordinates nursing staff at all sites to ensure that patients who begin their prenatal care at the site and receive an initial Nursing intake and education, appropriate initial labs, CSS intake, WIC referral, and initial provider visit in a timely manner.
  • Conducts OB intakes for all women who enter the High Risk OB Clinic.
  • Utilizes ascribed processes for managing the needs of high risk OB patients, initiating interventions based on physician-approved patient-specific protocols and order sets.
  • Contacts High Risk OB patients directly as needed to coordinate care, including postpartum care, reminder calls to patients when necessary, handling triage calls from High Risk OB patients, and follow up on test results as directed by the prenatal clinician.
  • Supports OB patients through the coordination of care for designated high risk OB patients. Serves as liaison for patients to other agencies involved in their care. Tracks and provides oversight of care delivery for non-high risk OB patients at all site as time frame of job allows.
  • Works with internal and external stakeholders to establish and maintain coordination of patient care services, to secure appropriate care and services within the health system and the community during the patients' pregnancy and post-partum period.
  • Ensures the development and maintenance of, constructive relationships with prenatal clinicians and care team members, including staff from all interfacing departments (Medical Services, Nursing Dept, CSS, Operations, Information Systems and other departments).
  • Interacts respectfully and collaboratively with patients and their families.
  • Ensures accurate and timely documentation of care coordination activities in patients' medical records.
  • Conducts regular chart reviews for High Risk OB patients to help ensure components of routine prenatal care are being accomplished, to identify missed care/gaps in care, and act to address these with prenatal clinician and patient. Conducts similar reviews for non-high risk OB patients as time frame of job allows.
  • Works closely with CCNM and CCNCs, in consult with medical leadership, to plan, develop and implement processes for maternal and child health care coordination, including workflows, data tracking, and a prenatal patient education program, to include written patient education materials and class offerings.
  • Facilitates the development and maintenance of a database/reporting capability on high-risk OB patients and utilizes these tools to identify and address missed care/gaps in care and determine if care coordination has improved patient status.
  • Participates in quality strategies to evaluate compliance with evidence-based guidelines and standards, identify opportunities to improve prenatal patient outcomes, and recommend and/or implement improvements to optimize care delivery and birth outcomes.
  • Facilitates a culture of a PCMH. Participates as a care team member around the care of high risk OB patients.
  • Engages in and facilitates other initiatives as assigned that align with the PCMH model.
  • Becomes knowledgeable about the GLHFC Improvement Plan and the model for improvement endorsed by the organization.
  • On a regular basis participate in ongoing quality improvement activities related to individual, team, and organizational performance improvement.
  • Performs all and any additional duties as assigned

Qualifications:

  • Active MA RN License
  • Minimum of Bachelor's degree in nursing or related field required, or actively pursuing said degree.
  • Current BLS for Healthcare Providers certification.
  • Minimum of 2 years nursing experience, preferably in perinatal health services or labor and delivery.
  • Familiarity with Latino population; Bilingual Spanish fluency required.
  • Knowledge of the organization and delivery of health care specific to prenatal care.
  • Ability to think critically and prioritize and organize patient care, with attention to detail.
  • Ability to work independently, and in partnership with other care team members.
  • Sound computer (eg. GE Centricity or other EMR, spreadsheet, word processing, database), analytical and organizational skills required.
  • Strong verbal and written skills required.

Pay: $45.00 - $50.00 per hour
