Positions are available for full-time, part-time, weekends. No experience in-home Health Care? –We will train! The RN is the “gatekeeper of care” and develops a patient care plan that is customized to each patient; working with the patient, the family, and the physician to best meet the care and wellness needs of each individual patient. Job Summary: Provides skilled nursing and case management to clients, following physicians orders and in coordination with other team members.

Reports To: Clinical Supervisor, Branch Manager, Branch President

Responsibilities:

  • Is compliant with all policies, procedures, and regulations set by the Federal and State governments, the agency, and other regulatory agencies, i.e. OSHA, Worker’s Compensation, licensure, and labor laws.
  • Coordinates client care with Case Managing RN, team members, and Clinical Supervisor.
  • Evaluates client nursing needs and develops a plan of care and goals appropriate to the findings, per physicians orders
  • Participates in in-services, meetings, and case conferences as required
  • Instructs client and family in the plan of care, and refers to community resources as necessary
  • Revises and updates nursing plan of care and goals as appropriate, with physician approval
  • Informs physician and Clinical Supervisor of changes in the client’s condition
  • Completes and submits all appropriate clinical documentation timely per agency policy
  • Maintains strict confidentiality of information relating to clients, employees, and the agency
  • Promotes customer satisfaction for all clients, employees, and visitors to the agency
  • Follows the agency’s Code of Ethics and Clients Rights
  • Approaches tasks with a positive and flexible attitude and practices team behaviors
  • Participates in after-hours, holiday, and weekend “on-call” rotations as assigned.
  • Performs other duties as assigned by the Clinical Manager or Branch President
  • Maintains continuing education as required by state licensure standards and provides documentation of same.

Job Requirements:

  • Knowledge of federal, state, and local regulations and agency policies and procedures
  • Competent in the provision of skilled nursing, including evaluation and assessment
  • Knowledge of community resources
  • Current CPR training/card; current professional liability insurance
  • Graduate of an accredited school of nursing (BSN preferred) and current licensure by the state of Colorado
  • 1 yr experience as a registered nurse, with home care experience preferred
  • Able to use a computer, telephone and voice mail, fax machine.
  • Able to use personal protective equipment, blood pressure cuff, stethoscope, venipuncture equipment, pulse oximeter, glucometer, infusion equipment and pumps
  • Able to demonstrate good written and oral communication skills, including writing or typing notes legibly
  • Able to travel to client’s homes and to work in unpredictable home environments
  • Able to lift 50 lbs unassisted and walk or navigate
  • Able to hear and observe client during treatment
  • Able to effectively teach client and family
  • Able to demonstrate team behaviors, effective time management skills, customer service skills, and leadership ability

Supervision:

Licensed Practical Nurses and field supervision of Certified Nurse Assistants and Personal Care Providers

Pay: $62,000.00 - $75,000.00 per year
