- Long Term Care Experience 1-2yr Required

Responsibilities:
- Identifies cognitive impairments, mood problems and other psycho-social needs in residents.
- Maintain accurate, thorough, and timely documentation.
- Review Advanced Directives and provide education to residents and family members as needed.
- Help provide a smooth admission and discharge process for residents and family members.
- Other duties as assigned by Administrator.
- Ability to work independently or part of a group.
- Computer knowledge.
- Ability to build and maintain rapport with residents and peers.
- Prior Long Term Care/Skilled Nursing Experience in Social Services or Required
- LSW Certificate required
