Are you a dedicated and compassionate Nurse Practitioner looking to make a meaningful impact in post-acute and long-term care? Complete Care Management is seeking an experienced Nurse Practitioner to join our team in Monmouth County, NJ. This role focuses on delivering high-quality, patient-centered care while working collaboratively with an interdisciplinary team.
Responsibilities:
- Conduct thorough patient assessments, diagnose conditions, and develop comprehensive care plans.
- Manage and treat complex medical conditions with a focus on reducing hospital readmissions.
- Document patient encounters in electronic health records (EHR).
- Collaborate with physicians, nursing staff, and other healthcare professionals to ensure seamless patient care.
- Communicate regularly with patients' families and support systems.
- Attend facility meetings and participate in care planning discussions.
- Maintain professionalism and uphold Complete Care’s commitment to excellence in patient care.
Qualifications:
- Master’s or Doctorate degree in Nursing from an accredited program.
- Active Nurse Practitioner (APN) License and Registered Nurse (RN) License in New Jersey.
- DEA, CDS, and BLS certifications.
- Minimum 2 years of Nurse Practitioner experience (post-acute care experience preferred).
- Strong clinical skills, including IV insertion, G-tube replacements, wound care, and suture/staple removal (preferred).
- Experience managing elderly patients and complex medical conditions.
- Proficiency in electronic health records (EHR).
