MDS Coordinator – PDPM Scrubber

Complete Care is seeking an experienced MDS Coordinator PDPM Scrubber to join our corporate team in Toms River, NJ. This position will play a key role in reviewing MDS assessments, clinical documentation, and PDPM classifications to ensure accuracy, compliance, and the ability to maximize appropriate PDPM reimbursement across our skilled nursing facilities.

The ideal candidate has strong hands-on MDS experience, a thorough understanding of PDPM, and a proven ability to identify opportunities to improve documentation and maximize reimbursement while remaining compliant with CMS guidelines.

Key Responsibilities of MDS Coordinator PDPM Scrubber:
  • Review MDS assessments and supporting clinical documentation for accuracy, completeness, and compliance.

  • Scrub MDS assessments to identify missed diagnoses, clinical conditions, services, and documentation opportunities that may impact PDPM reimbursement.

  • Analyze PDPM classifications and identify opportunities to maximize appropriate PDPM reimbursement.

  • Partner with facility MDS Coordinators, Directors of Nursing, therapy teams, and clinical leadership to resolve documentation and coding discrepancies.

  • Provide education and guidance to facility teams regarding MDS accuracy, PDPM, and documentation requirements.

  • Conduct ongoing reviews to identify trends, errors, and opportunities for improvement.

  • Assist with audits and ensure MDS processes align with CMS, RAI, and regulatory requirements.

Qualifications
  • MDS experience is required.

  • Minimum 2–3 years of hands-on MDS experience in a skilled nursing/post-acute setting required.

  • Strong working knowledge of MDS 3.0, RAI guidelines, PDPM, and Medicare reimbursement.

  • Demonstrated ability to review and maximize appropriate PDPM reimbursement through accurate clinical documentation and coding.

  • Experience auditing MDS assessments and identifying reimbursement opportunities.

  • Must be able to work on-site at the Complete Care corporate office in Toms River, NJ.
