Complete Care Management is seeking an experienced and driven External Care Manager (ECM) to represent our network of skilled nursing and post-acute facilities across Union and Essex Counties. This role focuses on building strong hospital relationships and supporting safe, smooth transitions of care from Overlook Medical Center and RWJ Rahway into Complete Care centers.
Key Responsibilities
- Serve as the on-site liaison between hospital discharge planners, case managers, and Complete Care facilities.
- Assess patient needs, coordinate placement, and facilitate timely admissions from hospital to post-acute settings.
- Educate hospital teams on Complete Care's continuum of services, clinical specialties, and post-discharge resources.
- Communicate daily with facility admission teams to ensure accurate, efficient patient transitions.
- Track referral activity, identify trends, and collaborate with internal leadership to strengthen referral partnerships.
- Participate in community outreach and hospital meetings to represent Complete Care professionally and proactively.
Qualifications
- Background in nursing, social work, case management, or healthcare marketing strongly preferred.
- Experience in hospital liaison, external care coordination, or admissions within skilled nursing/post-acute settings required.
- Excellent communication, relationship-building, and organizational skills.
- Ability to work independently and travel between hospital sites and local facilities.
- Current NJ driver's license and reliable transportation required.
