Complete Care is seeking an experienced and motivated External Care Manager (ECM) to join our growing team in the Chicago region. This position will focus on building and maintaining strong relationships with hospitals, physicians, and community partners to support smooth transitions of care into our skilled nursing facilities.
Our Chicago region includes three Complete Care locations and works closely with major health systems such as Rush, UI Health, Mt. Sinai, and Loyola.
Key ResponsibilitiesDevelop and maintain strong referral relationships with hospital discharge planners, case managers, and social workers.
Coordinate admissions and ensure seamless transitions from acute care hospitals to Complete Care facilities.
Communicate clinical and logistical details efficiently between hospital partners and facility teams.
Represent Complete Care professionally during on-site hospital visits, care conferences, and community outreach events.
Track and report referral activity, admission trends, and opportunities for growth.
Work collaboratively with internal admissions, marketing, and clinical teams to meet census and quality goals.
Experience in healthcare marketing, hospital liaison, case management, or discharge planning (preferably in post-acute/skilled nursing).
Strong understanding of hospital systems and patient flow processes.
Excellent interpersonal and communication skills with a relationship-focused approach.
Ability to travel between facilities and hospital partners within the Chicago region.
Licensed nurse, social worker, or healthcare professional background preferred but not required.
