Now Hiring: LSW Social Work Assistant
Responsibilities:
- Identifies cognitive impairments, mood problems and other psycho-social needs in residents.
- Maintain accurate, thorough, and timely documentation.
- Review Advanced Directives and provide education to residents and family members as needed.
- Help provide a smooth admission and discharge process for residents and family members.
- Other duties as assigned by Director of SW or facility Administrator
Qualifications:
- Prior experience managing staff is preferred.
- Ability to work independently or part of a group.
- Computer knowledge.
- Previous long term care experience is preferred but not required
License:
- Must have active Bachelors Degree for Social Work/LSW
