Position Summary
We are seeking a skilled and compassionate Registered Nurse Case Manager to join our interdisciplinary team in the Brighton area. This role is central to planning, coordinating, and delivering hospice care in alignment with each patient’s individualized plan of care. You’ll work closely with patients, families, physicians, and team members to ensure dignity, comfort, and continuity throughout the end-of-life journey.
Key Responsibilities
- Develop and implement individualized hospice care plans
- Conduct initial and ongoing comprehensive assessments
- Perform physical exams and obtain medical histories
- Monitor vital signs, administer medications, and evaluate treatment responses
- Educate patients and families on the dying process, care options, and health decisions
- Provide dietary counseling and emotional support
- Coordinate services with the IDG and contracted providers
- Consult with attending physicians and Hospice Medical Director regarding care changes
- Supervise Hospice Aides
- Participate in IDG meetings, case conferences, QAPI activities, and staff development
- Maintain accurate, timely clinical documentation in compliance with HIPAA
- Submit weekly visit schedules and ensure plans of care remain current
- Perform death visits and support discharge planning
- Uphold Medicare, Medicaid, and accreditation standards
- Serve on-call shifts including holidays, per agency policy
- Collaborate with external agencies to ensure continuity of care
