Role Overview
As a Registered Nurse (RN) within our hospice care team, you will play a vital role in delivering holistic, patient-centered end-of-life care. This position emphasizes symptom management, emotional support, and advocacy—ensuring each patient’s final chapter is met with dignity, comfort, and clarity. You’ll coordinate care plans, guide families through complex decisions, and collaborate with a multidisciplinary team to optimize outcomes and uphold our commitment to compassionate care.
Key Responsibilities
- Conduct thorough assessments of patients’ medical, emotional, and psychosocial needs
- Develop and manage individualized care plans that reflect patient goals and clinical best practices
- Monitor vital signs and administer medications in accordance with physician orders and hospice protocols
- Provide education and emotional support to patients and families regarding end-of-life options and symptom management
- Coordinate care with interdisciplinary team members—including physicians, social workers, chaplains, and home health aides—to ensure seamless, comprehensive support
- Maintain accurate and timely documentation in compliance with HIPAA, CMS, and state hospice regulations
- Advocate for patients’ needs, preferences, and rights throughout their care journey
Required Qualifications
- Active RN license in the state of Michigan
- Minimum of 2 years’ nursing experience, with at least 1 year in hospice or palliative care preferred
- Proficiency in EMR systems and clinical documentation
- Valid driver’s license and reliable transportation for home visits
