Home Health Intake Coordinator

We are seeking a dedicated and detail-oriented Intake Coordinator to join our growing team. The ideal candidate will have prior experience in home health or healthcare administration and possess excellent communication and organizational skills.

Responsibilities

• Receive and process new home health referrals from hospitals, physicians, and other referral sources.
• Verify insurance eligibility, authorizations, and coverage for services.
• Ensure all required documentation and clinical information are obtained prior to admission.
• Coordinate start-of-care scheduling with clinical staff, patients, and caregivers.
• Communicate efficiently with case managers, physicians, and internal team members.
• Maintain compliance with agency policies, HIPAA, and Medicare/Medicaid regulations.
• Accurately enter patient and referral data into EMR systems (Kinnser/WellSky experience preferred).
• Track referral status and follow up on pending authorizations and documentation.
• Provide administrative support to ensure seamless patient intake and timely admissions.

Qualifications & Requirements

• Minimum 1–2 years’ experience in a home health intake or healthcare office setting required.
• Knowledge of Medicare, Medicaid, and private insurance authorizations.
• Experience with Kinnser/WellSky EMR strongly preferred.
• Strong multitasking, organizational, and communication skills.
• Ability to work in a fast-paced environment with attention to detail.
• Professional phone etiquette and customer service skills.
• High school diploma or equivalent (Associate’s or Bachelor’s degree preferred).
• Bilingual (English/Spanish or Creole) a plus.
