Job Summary
We are seeking a compassionate and dedicated Personal Care Aide - Client Care Assistant to join our team and provide exceptional support to individuals in various care settings. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patients' vital signs, health status, and behavioral changes in accordance with care plans
- Support medication administration following HIPAA guidelines and care protocols
- Prepare nutritious meals and assist with meal planning to meet individual dietary needs
- Maintain a clean and safe environment through regular cleaning, sanitation, and organization of living spaces
- Use equipment such as Hoyer lifts or other assistive devices to safely transfer clients with mobility challenges
- Observe and report any changes in patient condition or behavior to healthcare professionals promptly
Skills
- Proven experience in home & community care, assisted living, nursing homes, or group homes setting
- Knowledge of Alzheimer's care, dementia care, memory care, and hospice care practices
- Ability to work effectively with people with developmental disabilities, autism, or other special needs
- Strong understanding of patient observation techniques and vital signs monitoring
- CPR and First Aid certification required; additional certifications such as HIPAA compliance preferred
- Experience with medication administration and creating or following care plans
- Excellent communication skills for working collaboratively with clients, families, and healthcare teams
- Physical ability for heavy lifting and assisting with mobility; comfortable working in home health environments
