Home Care Aide

Job Summary
We are seeking compassionate and dedicated Home Care Aides to provide exceptional in-home support to clients in need of assistance with daily activities and health management. This vital role involves delivering personalized care that promotes independence, safety, and well-being. As a Home Care Aide, you will work closely with clients, families, and healthcare teams to ensure high-quality care in a respectful and empathetic manner. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
• Monitor and observe patients for changes in condition, vital signs, and overall health status, documenting findings accurately in electronic health record (EHR) or electronic medical record (EMR) systems
• Support medication administration following medical documentation protocols while ensuring compliance with HIPAA regulations for patient privacy
• Provide companionship and emotional support, especially for individuals with dementia or Alzheimer’s disease, fostering a safe and engaging environment
• Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
• Assist with medical procedures including catheter care, wound care, and hospice-related tasks as directed by healthcare professionals
• Maintain detailed records of patient care activities, medical documentation, and updates on patient progress for seamless communication with healthcare teams

Skills

• Strong knowledge of patient observation techniques and dementia or memory care practices
• Experience working in assisted living facilities, nursing homes, or home health environments with individuals who have developmental disabilities or disabilities
• Familiarity with EMR/EHR systems and medical terminology to accurately document care activities
• Ability to perform CPR and handle emergency situations confidently while maintaining composure
• Competence in assisting with ADLs such as meal preparation, medication management, catheter care, and vital signs monitoring
• Experience providing hospice care or working with patients requiring long-term or palliative care is highly valued
• Excellent caregiving skills combined with compassion, patience, and strong communication abilities to foster trust and rapport with clients

Requirements

• Previous experience in home health, senior care, or memory care preferred.
• Knowledge of first aid practices is an asset.
• Ability to prepare meals that meet dietary restrictions and preferences.
• Compassionate nature with excellent interpersonal skills.
• Strong organizational skills and attention to detail.
• Ability to work independently as well as part of a team.
• Must be able to lift or assist clients as needed.
• A valid driver's license may be required based on client needs.
