Job Summary
We are seeking compassionate and dedicated Home Care Aides to provide exceptional in-home support to clients in need of assistance with daily activities and health management. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and observe patients for changes in condition, vital signs, and overall health status, documenting findings accurately in electronic health record (EHR) or electronic medical record (EMR) systems
- Support medication administration following medical documentation protocols while ensuring compliance with HIPAA regulations for patient privacy
- Provide companionship and emotional support, especially for individuals with dementia or Alzheimer’s disease, fostering a safe and engaging environment
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
- Assist with medical procedures including catheter care, wound care, and hospice-related tasks as directed by healthcare professionals
- Maintain detailed records of patient care activities, medical documentation, and updates on patient progress for seamless communication with healthcare teams
Skills
- Strong knowledge of patient observation techniques and dementia or memory care practices
- Experience working in assisted living facilities, nursing homes, or home health environments with individuals who have developmental disabilities or disabilities
- Familiarity with EMR/EHR systems and medical terminology to accurately document care activities
- Ability to perform CPR and handle emergency situations confidently while maintaining composure
- Competence in assisting with ADLs such as meal preparation, medication management, catheter care, and vital signs monitoring
- Experience providing hospice care or working with patients requiring long-term or palliative care is highly valued
- Excellent caregiving skills combined with compassion, patience, and strong communication abilities to foster trust and rapport with clients
