THIS IS FOR THE HEMET, MURRIETA, MENIFEE AREAS:
We are seeking a compassionate and dedicated Registered Nurse Case Manager for our Hospice program. This role is essential in providing high-quality patient care and support to individuals and their families during end-of-life transitions. The ideal candidate will possess a strong background in nursing with experience in critical care, geriatrics, and case management. You will be responsible for coordinating patient care, conducting assessments, and ensuring compliance with healthcare regulations.
Duties
- Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
- Coordinate with multidisciplinary teams to implement effective patient service strategies.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life decisions.
- Maintain accurate medical records in compliance with HIPAA regulations and organizational policies.
- Facilitate discharge planning and transition of care from inpatient to outpatient settings.
- Conduct intake evaluations and diagnostic assessments for new patients.
- Utilize knowledge of medical terminology, physiology, and nursing practices to deliver high-quality care.
- Engage in utilization management activities to ensure appropriate resource allocation.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience working with Hospice/Terminally ill patients
- Experience with Hospice Admissions a plus!
- Excellent communication skills for effective interaction with patients, families, and healthcare team
- Ability to work independently as well as collaboratively within a team environment.
- Reliable transportation

Pay: From $70.00 per hour
Expected hours: 5 – 20 per week
