Job Description – Referral Coordinator / Community Outreach (part-time). This position requires excellent communication skills working for a non-medical home health care agency.
The objective is to generate geographically appropriate, referral-driven private-pay cases that support block scheduling and caregiver retention. Industry experience is preferred but not required
The core responsibilities include:
- Developing relationships with hospital discharge planners
- Skilled nursing/rehab facilities
- Elder law attorneys
- Geriatric care managers
- Financial planners and wealth managers
Hours & Structure
- 10-15 hours/week
- Flexible schedule
- Hourly pay bonus tied to hours of care started
Pay: $24.21 - $29.15 per hour
