Duties
- Provide person-centered support to help individuals stay safe, independent, and engaged
- Assist with mobility and transfers (use equipment as trained; ensure fall prevention)
- Support activities of daily living: hygiene, dressing, grooming, toileting (as needed)
- Help with meal preparation, nutrition reminders, and light housekeeping
- Encourage skill-building and independence in daily routines
- Support community participation (appointments, outings, errands, social activities)
- Offer companionship, conversation, and emotional support
- Monitor and report changes in health, behavior, or safety
- Complete accurate, timely documentation (progress notes, outcomes, incidents)
- Follow each person’s service plan, risk plans, and provider policies
- Use respectful communication and support dignity, privacy, and choice
- Work collaboratively with supervisors, care teams, and families when appropriate
- Maintain a safe, organized living environment
- Follow MN 245D requirements and positive support practices
Requirements
- Prior experience working in assisted living facilities, group homes, nursing homes, or home & community care settings is highly preferred
- Compassionate, reliable, and patient
- Ability to follow care plans and complete documentation
- Able to pass a background check and required trainings
- Reliable transportation for shifts and appointments
- Comfortable assisting with transfers and mobility (training provided)
