Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient health status, recording vital signs such as blood pressure, pulse, temperature, and respiration
- Administer medications and treatments as prescribed under medical supervision while maintaining accurate medical documentation
- Support patients with mobility transfers using patient transfer methods and equipment like Hoyer lifts or gait belts
- Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
- Maintain patient confidentiality in accordance with HIPAA regulations and ensure compliance with home healthcare regulations
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment
Requirements
- Proven experience working with individuals with disabilities or in assisted living or nursing home settings
- Knowledge of medical terminology, medication administration, catheter care, and end-of-life care practices
- Ability to operate EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation
- Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications such as hospice or dementia care are a plus
- Strong communication skills for effective client interaction and collaboration with healthcare teams
- Physical ability to perform heavy lifting, assist with patient transfers, and provide physical support as needed
- Familiarity with home health care assistance standards and home & community care experience
- Experience working with geriatric patients or individuals with developmental disabilities is highly desirable
