Managers for a Managed Care Plan servicing New York. The RN Care Manager position works as a Hybrid - Remote Case Manager, working primarily from home, with 1 day of Field Case Management visits per week, in designated areas. Bilingual Spanish, Chinese and Bengali needed for Manhattan, Bronx or Brooklyn visits.
Qualified candidates will be able to work from their home and will make Field visits 1 day per week in field. Qualified candidates will have prior RN Case Management experience within Managed Care, CHHA Home Care or Hospital Case Management, bilingual language ability a plus. Qualified candidates will have a NYS RN License or NYS NP, and prior RN Case Management, Coordination of Care experience within a multi-disciplinary team.
Patient visit areas available are: Bilingual Spanish in Manhattan or Bronx visits, Bilingual Chinese in Brooklyn or Upper Manhattan and Bilingual Bengali in the Bronx.
Remote / Hybrid - RN Care Manager - Bilingual - Responsibilities:
Reporting to the Supervisor of Care Management, this position will provide telephonic and in-person case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the member's care plan in conjunction with the PCP, caregivers and other team members in order to help meet the member's needs. managed care nurse, managed care rn, nursing, rn, nurse, care management
Qualifications:
NYS RN license required
CCM preferred
At least 2 years of RN Case Management experience in managed care, home care, acute care, hospital or nursing home preferred
Ability to make patient visits 1 day per week, ability to work remote from home the remaining days (In Office during Probation period, remote work begins after Probation period)
Reliable transportation

Bilingual Spanish, Bilingual Chinese (Cantonese/Mandarin) & Bilingual Bengali & English in demand!
Prior experience in an interdisciplinary service delivery environment, required.
Working knowledge of Medicare and Medicaid regulations
Excellent Salary $105,000 - 115,000 based on relative experience and great Benefits!
