Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team for nursing home rounding. The ideal candidate will possess a strong foundation in patient care within long-term care settings, demonstrating expertise in clinical assessments, medication administration, and patient advocacy. This role offers an opportunity to provide compassionate, high-quality nursing services to our senior residents, ensuring their comfort and safety while adhering to regulatory standards.
Responsibilities
- Collaborate with interdisciplinary teams to develop individualized care plans tailored to each resident’s needs.
- Maintain detailed medical records within electronic health record (EHR) systems, ensuring compliance with HIPAA and long-term care regulations.
- Educate residents and their families about health conditions, medication management, and safety protocols.
- Monitor residents’ condition changes during rounds, promptly reporting concerns or abnormalities to supervising healthcare providers.
Experience
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Prior experience working in nursing homes or skilled nursing facilities is highly preferred.
- Knowledge of long-term care regulations, including documentation standards and resident rights.
- Experience with EMR/EHR systems such as Epic or Cerner is advantageous.
- Familiarity with geriatric care principles, dementia care including Alzheimer’s and memory support, and chronic disease management.
- Strong communication skills for effective caregiver communication and patient education.
