Key Responsibilities
- Conduct comprehensive assessments and ongoing reassessments to identify individual strengths, needs, and service opportunities
- Develop and update person-centered care plans that reflect individual goals, preferences, and progress
- Coordinate and connect individuals and families to appropriate services, supports, and community resources
- Monitor service delivery to ensure quality, effectiveness, and alignment with individual goals
- Maintain timely, accurate documentation in accordance with agency standards
- Collaborate with interdisciplinary teams to review progress, evaluate services, and recommend adjustments
- Travel to various locations to complete assessments and provide ongoing support
Education, Skills, Knowledge and Expertise
- Bachelor's degree* or Nursing Diploma/Associate degree in nursing required
(*in a human service-related field preferred)
or - Master's degree in psychology, social work, rehabilitation/vocational/pastoral counseling, family therapy or closely related field required
Work Experience/Training/Additional Requirements:
- Experience (lived or trained) with individuals with developmental disabilities required if bachelor's degree is not in a human service-related field
- A valid driver's license and agency established minimum automobile coverage required
LICENSURE/CERTIFICATION:
- RN license required if a nursing degree is obtained
