Compensation: $25.54 - $26.81 / hour

As a Support Coordinator, you will play a vital role in empowering individuals with developmental disabilities to live full, self-directed lives. Through person-centered planning and ongoing coordination, you'll help connect individuals and families to essential medical, social, and community resources while promoting independence, choice, and overall wellness.

Key Responsibilities
  • Conduct comprehensive assessments and ongoing reassessments to identify individual strengths, needs, and service opportunities
  • Develop and update person-centered care plans that reflect individual goals, preferences, and progress
  • Coordinate and connect individuals and families to appropriate services, supports, and community resources
  • Monitor service delivery to ensure quality, effectiveness, and alignment with individual goals
  • Maintain timely, accurate documentation in accordance with agency standards
  • Collaborate with interdisciplinary teams to review progress, evaluate services, and recommend adjustments
  • Travel to various locations to complete assessments and provide ongoing support


Education, Skills, Knowledge and Expertise
  • Bachelor's degree* or Nursing Diploma/Associate degree in nursing required
    (*in a human service-related field preferred)
    or
  • Master's degree in psychology, social work, rehabilitation/vocational/pastoral counseling, family therapy or closely related field required


Work Experience/Training/Additional Requirements:


  • Experience (lived or trained) with individuals with developmental disabilities required if bachelor's degree is not in a human service-related field
  • A valid driver's license and agency established minimum automobile coverage required


LICENSURE/CERTIFICATION:
  • RN license required if a nursing degree is obtained
