We're looking for a Support Coordinator to join our team. Someone who’s not just experienced, but truly committed to supporting people in leading safe, healthy, and fulfilling lives at home and in their communities.
What You’ll Need to Be Successful:
- A Bachelor’s degree in a human services field (psychology, social work, sociology, or similar)
- At least 2 years of experience working with individuals with developmental and/or intellectual disabilities (DD/IDD) or related conditions
- 2 years of experience in a healthcare or social services setting
- Preferred experience in case management—you understand how to coordinate supports, advocate for services, and document clearly
What You’ll Be Doing:
- Building trusted relationships with individuals and their families
- Conducting in-person visits in homes and community settings
- Facilitating service planning (ISPs) meetings, provider selection, and referrals
- Monitoring progress and ensuring services are truly meeting the person’s needs
- Keeping accurate records and documentation
- Collaborating with providers and stakeholders to problem-solve and advocate
This role requires regular travel within Gwinnett, Fulton, Barrow counties , and surrounding counties, and a reliable vehicle is a must. While many tasks can be done remotely, you’ll be in the field often.
