Targeted Case Manager/Wrap Around Facilitator
Full Time
GENERAL RESPONSIBILITIES:
- Provide Target Case Management and Wraparound Facilitation to persons assigned to caseload.
- Facilitate meetings with treatment/wraparound teams.
- Develop comprehensive treatment plans for persons in service.
- Coordinate, monitor and/or complete all waiver paperwork, releases, and assessments.
- Work hours are flexible to meet the needs of the person and families served.
- Monitoring of progress of treatment plans and interventions and making changes as necessary.
- Coordination of care with community partners.
ESSENTIAL JOB FUNCTIONS:
- Complete treatment plans every 90 days for persons in service.
- LOC reviews completed every 90 days.
- Updates to treatment plans provided to treatment team in a timely manner.
- Care coordination with community partners occurs at minimum every 90 days.
- Progress on interventions and services in treatment plans monitored monthly.
- Waiver paperwork, treatment plans, releases and all other CBS related paperwork/forms completed timely and thoroughly.
- Progress notes complete within 48 hours of service.
- EMR scheduler is utilized and kept updated.
Qualifications:
MINIMUM QUALIFICATIONS:
- Age 21 years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience.
- Excellent interpersonal and teamwork skills
- Excellent organizational and time management skills
- Interest and investment in working with children who experience Severe and Emotional Disturbance (SED) and their families.
- Have completed or will complete any required training within six months of employment.
- Must have a current and valid driver’s license.
Salary : $22 - $28
