Position Summary:	This position plays a vital role in supporting women and their children throughout their recovery journey at Bridgehouse. This role supports patients from admission through discharge planning and transition. The Recovery Navigator assists with care coordination, provides child supervision during group sessions, and participates in an after-hours on-call rotation.
Essential Job Functions:	Serve as a consistent recovery support presence beginning at admission, helping patients feel welcomed and oriented to the program. Assist in developing and implementing discharge plans in collaboration with the care team, ensuring continuity of support after patients leave Bridgehouse. Support patient access to critical services, including healthcare, peer support, transportation, housing, and community resources. Provide safe and nurturing supervision of children during on-site group programming. Track and document patient progress, engagement, and appointment follow-through. Participate in a rotating on-call schedule (1–2 weeks per month) to respond to patient concerns outside of regular hours. Maintain accurate and timely documentation of all support activities. Foster a welcoming, safe, and recovery-focused atmosphere for both patients and their children. Engage in team meetings, required training, and regular supervision. Comply with all CKF policies and procedures and related clinical licensure standards. Other duties as assigned.
Physical Requirements: Based on an 8-hour day	Sit – Occasionally - .25 – 2.5 hrs daily
Stand - Occasionally - .25 – 2.5 hrs daily
Lift/Carry 0-10 lbs. – Occasionally - .25 – 2.5 hrs daily
Walk – Occasionally - .25 – 2.5 hrs daily
Reach Above - Occasionally - .25 – 2.5 hrs daily
Bend/Stoop - Occasionally - .25 – 2.5 hrs daily
Use of hands – Pushing/Pulling - Occasionally - .25 – 2.5 hrs daily
Use of hands – Fine Manipulation - Continuously – 5.5 – 8 hrs daily
Stress Level - High
PHI Access Level:	Level 2 – This position will have regular access to patient records, no access to employee medical information.
Work Environment:	On-Site
