Responsibilities
- Provide assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals
- Assist with medication administration following prescribed instructions while maintaining accurate medical documentation
- Support patients with specialized needs including dementia care, Alzheimer’s care, hospice care, and working with individuals with developmental disabilities or disabilities
- Utilize Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient information securely in compliance with HIPAA regulations
- Prepare nutritious meals and assist with meal planning to promote health and safety
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment in accordance with home & community care standards
- Document medical records accurately and ensure confidentiality at all times
- Collaborate effectively with nursing staff and other healthcare providers to deliver comprehensive patient care in home settings
Skills
- Strong patient observation skills to detect changes in condition or behavior
- Experience working in assisted living facilities, nursing homes, or home health environments
- Knowledge of medical terminology and procedures related to home health care
- Proficiency with EMR/EHR systems for medical documentation and record keeping
- Ability to assist with ADLs including bathing, dressing, transferring, and feeding patients
- Familiarity with specialized care such as dementia, Alzheimer’s disease, hospice support, catheter care, and medication administration
- CPR certification and knowledge of emergency response procedures
- Experience operating mobility aids like Hoyer lifts safely and effectively
- Compassionate approach to caring for seniors and individuals with disabilities or long-term health conditions
- Excellent communication skills for working with patients, families, and healthcare teams
- Ability to handle sensitive situations discreetly while maintaining patient dignity
