Job Summary:
Responsible for providing professional nursing care to the agency’s home care clients as prescribed by the physician and in compliance with the state’s Registered Nurse Practice Act, any applicable licensure/certification requirement and agency policies and procedures.
Responsibilities:
- Use professional nursing skills of assessment, planning, implementation and evaluation in working with client/family.
- Identify present and potential needs/resources in relation to the health of client/family. Any needs for waiver clients are reported to the Service Coordinator/Care Manager.
- Demonstrate ability to analyze information received via assessment process.
- Attend team meetings per the request of the Service Coordinator/Care Manager for waiver clients. Communicate and complete any required reporting to insurance carriers/MLTC care managers for non-waiver clients.
- Revise written care plan based on assessed needs and goals set mutually with client/family and nurse. For waiver clients only, write and implement a Detailed Goal Plan within 30 days of the start of care, place a copy in the home, and ensure goals are added to the plan of care in the home.
- Develop and review the Detailed Goal Plan with the client, and submit it every 6 months with an Individualized Service Report to the Service Coordinator/Care Manager as part of every 6 month team meeting.
- Administer and prefill medication, provide medical treatment ordered by the physician for all non-waiver clients.
- Perform the RN role in developing abstracts for clients seeking HCSS in the waiver programs.
- Perform agency requested “meet and greets” to ensure appropriateness and ability to provide a safe plan of care in the home environment.
- Supervise HCSS staff working with clients in their homes. Oversee PCA or HHA level of aide services for any nonwaiver clients.
- Teach/train HCSS, PCA, or HHA staff on proper use of durable medical equipment or other care aids.
- Recommend and initiate appropriate preventative, therapeutic and rehabilitative procedures and techniques to clients as appropriate.
- Evaluate care plan for progress towards goals and revise as needed.
- Demonstrate awareness of and adherence to waiver, non-waiver, and agency policies and procedures ensuring safe and skilled nursing care.
- Perform client’s personal care and/or assist with activities of daily living as needed.
- Ensure timely and proper completion of all agency medical related documents and ensure that all documentation meets NYSDOH regulations and agency standards.
- Provide after hours on-call services to ensure oversight and safety for aide staff and clients.

Position Requirements:
- Graduate of an accredited School of Nursing.
- Valid and Current New York State Registered Nurse License and Registration.
- Minimum of one (1) year medical/surgical experience.
- Previous Home Care experience preferred.
- Ability to successfully complete a Competency & Proficiency Test & Skills Evaluation appropriate to job title before hire.
- Excellent verbal and written communication skills.
- Ability to function with minimal direct supervision, to make decisions, to problem solve, and to handle emergencies.
- Ability to relate to a diverse and multi-cultural population.
- Pass a physical examination and immunization requirements after being offered employment, and a health assessment and PPD annually thereafter.
- Possess a valid driver’s license and have a car available for use.
- Complete CCOR’s orientation program.
- Able to lift up to 25 pounds; able to sit, stand, bend, stoop, pull, reach, lift, and perform repetitive movements of the upper extremities.

Work Hours:
- As scheduled by supervisor to facilitate 7 day a week coverage (includes weekend and holiday rotation)
