Position Overview

The Housing Case Manager provides individualized housing support services and case management to participants enrolled through NJ FamilyCare. This position works closely with Managed Care Organizations (MCOs), community partners, landlords, and participants to secure and maintain safe, affordable, and stable housing.

The Housing Case Manager assists participants in navigating housing resources, advocating for their needs, and promoting long-term housing stability while supporting overall health, wellness, and independence. This role requires strong advocacy skills, knowledge of housing systems, and a commitment to serving vulnerable populations throughout New Jersey.

Key Responsibilities

  • Provide housing-focused case management and support services to eligible participants in accordance with NJ FamilyCare guidelines
  • Collaborate with MCO care managers, community agencies, landlords, and local housing authorities to identify appropriate housing opportunities
  • Conduct housing assessments and develop individualized housing service plans tailored to participant needs and goals
  • Assist participants with housing applications, lease agreements, and other required documentation
  • Support participants with housing retention skills, including budgeting, tenancy education, conflict resolution, and independent living skills
  • Advocate on behalf of participants with landlords, property managers, and community organizations to address barriers to housing
  • Conduct monthly in-person meetings, follow-up appointments, and wellness check-ins based on participant level of need
  • Provide crisis intervention and connect participants to emergency housing resources when necessary
  • Maintain accurate, timely, and compliant documentation in accordance with agency, MCO, and state requirements
  • Participate in interdisciplinary team meetings, case conferences, and collaborative planning with MCOs and community providers
  • Stay informed regarding housing programs, subsidies, landlord/tenant regulations, and policy updates affecting participants

QualificationsRequired Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field required
  • (Master of Social Work preferred)
  • Minimum of three (3) years of experience in case management, housing support services, or working with individuals with disabilities or vulnerable populations
  • Knowledge of affordable housing programs, landlord/tenant laws, and community resources throughout New Jersey
  • Excellent interpersonal, communication, advocacy, and problem-solving skills
  • Strong organizational skills with the ability to manage multiple cases and deadlines effectively
  • Proficiency in Microsoft Office and electronic health record (EHR) systems
  • Valid New Jersey Driver’s License, reliable transportation, and willingness to travel locally for participant visitsPreferred Qualifications
  • Experience collaborating with Managed Care Organizations (MCOs)
  • Bilingual English/Spanish
  • Experience working within NJ FamilyCare or supportive housing programs
  • Knowledge of trauma-informed care practices

Pay: $25.16 - $30.30 per hour
