What You’ll Do
- Identify and engage hard-to-reach patients, addressing health and social needs through trust-building and support.
- Connect patients to healthcare, community resources, and benefit programs while reducing barriers to care.
- Collaborate with providers, case managers, and agencies to coordinate services and ensure smooth care transitions.
- Coach patients toward health goals and assist with appointments, applications, transportation, and home visits.
- Advocate for vulnerable populations while maintaining flexibility, cultural sensitivity, and continuous learning.
What We’re Looking For
- BLS Certificate from American Red Cross or American Heart Association
- Clean driving record with current NC driver’s license and approved by corporate vehicle insurance vendor for driving privileges
- High School Diploma or equivalent
- (1) one year experience working with the public or in customer service; in medical setting
- Community Health Worker Certification preferred
