Case Manager Community Based

MAIN FUNCTION:            Liaison with community and other health facilities

REPORTS TO:                    Program Manager

DUTIES AND RESPONSIBILITIES:

• Will conduct pre-admission visits on prospective residents for the Presbyterian Residential Community, ACF and ALP units.
• Will perform initial and periodic evaluations, at least once every twelve months, of the needs of residents and the capability of the facility program to meet those needs.
•  Will orient new residents and their family members to the daily routine and will assist residents to adjust to life in the facility.
•  Will assist residents to maintain family and community ties and encourage resident participation in facility and community activities.
•  Will arrange for services from public and private sources for income, health, mental health and social services and will assist residents in making application for and maintaining \income entitlements and public benefits.
•  Will assist residents in obtaining and maintaining a primary physician or medical care for management of individual needs.
•  Will coordinate and follow through on all necessary paperwork to assure compliance with all the rules and regulations as mandated by DSS and DOH.
•  Will assist residents in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the residents health or  mental health, including health and mental health services, dental services and medications.
•  Will provide information and referral according to resident needs.  Will assist residents in need of alternative living arrangements to make and execute sound  discharge or transfer plans.
•  Will assist residents to participate in facility operations with recommendations or grievances.  Will be liaison for resident council.
•  Will conduct tours of the facility.  Will handle marketing duties of the facility and participate in speaking engagements on behalf of the facility.
•  Will assist residents with internal or external services as necessary.

• EDUCATION:     
  • An associate degree in the human services field is required 
  • Social Work experience preferred but not required.  
  • Bachelor’s Degree preferred, but not required
• CONTACTS:        Daily contact with residents and the professional and non-professional members of the community.

Salary : $20 - $22
