Job Overview
We are seeking a dedicated and compassionate Case Manager to join our nonprofit. The Case Manager will work closely with clients to assess their needs, develop personalized care plans, and facilitate access to appropriate services.
Responsibilities
- Conduct comprehensive assessments of clients' needs and strengths.
- Develop and implement individualized service plans that address clients' goals.
- Provide crisis intervention and management to clients in urgent situations.
- Collaborate with community resources, including childcare services, child welfare agencies, and housing programs such as Section 8.
- Monitor client progress and adjust care plans as necessary to ensure effective outcomes.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
- Perform outreach in community and encampment settings
Qualifications
- Bachelor’s degree in social work, psychology, or a related field; Master’s degree preferred.
- Experience in workforce development or related fields is highly desirable.
- Knowledge of behavioral health systems and crisis intervention techniques.
- Familiarity with child welfare policies and practices is a plus.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent organizational skills and attention to detail for managing multiple cases effectively.
- Ability to work independently as well as collaboratively within a team environment.
- Proficient in Microsoft Office Suite and case management software.
- Engage and establish rapport with individuals experiencing homelessness.
Requirements
- High School Diploma or equivalent required; Bachelor's degree in Social Work, Psychology, Sociology, or related field preferred.
- Knowledge of homelessness and housing resources in the local area.
- Excellent communication skills for effective interaction with clients and stakeholders.
- Flexible schedule availability for engagement and rapport-building.
- Client-centered, trauma-informed approach to services.
- Crisis intervention skills and ability to de-escalate conflicts.
- Empathy and compassion for individuals experiencing homelessness or housing insecurtiy.
- Ability to work collaboratively with shelter staff and external agencies.
- Proficiency in Microsoft Office (Word, Excel, Outlook).
- Have reliable transportation
- Conduct assessments to identify immediate needs and eligibility for services.
- Connect individuals with available resources such as housing options and emergency shelters.
- Assist clients with applications for financial assistance and benefits.
- Refer clients to healthcare services for medical issues or chronic conditions.
- Document outreach interactions using mapping software and HMIS.
- Provide ongoing support to individuals in previously cleared areas.
- Prepare detailed reports on housing placements and services offered.
- Collaborate with community members and leaders to facilitate cooperation on housing options.
- Attend CoC meetings and other required meetings.
- Evaluate individuals applying for homelessness prevention or rapid re-housing assistance.
- Develop comprehensive plans to meet the specific needs of program participants.
- Monitor and evaluate participants' progress towards housing stability.
- Maintain high ethical standards and respect client confidentiality.
- Respond promptly to crisis situations and provide immediate assistance.
- Notify the manager of any work orders or supplies needed.
- Attend staff meetings and complete training as required.
