PRIMARY ROLE: Responsible for providing direct assistance and support to persons in day services. Services are described by the individual plan to increase independence, improve status in the community and overall increase the quality of life. Direct Support Professionals are assigned to a residence and/or other sites within CVS and the community depending on the operational/program needs. Direct Support Professionals must be able to work effectively with or without direct supervision, work cooperatively within a team setting, handle crisis situations calmly and proficiently and interact with others in a courteous and professional manner. Must be able to adapt to the flexible nature of the job and work in a busy, fast-paced environment.

• Must display empathy, compassion, and treat all staff and individuals supported with dignity and respect.
• Responsible for ensuring the health and safety needs of the individuals are always maintained through continuous monitoring.
• Responsible for attending mandatory in-service trainings and scheduled staff meetings.
• Report incidents of abuse and neglect in accordance with DDS Standards of Practice Guidelines.
• Responsible for all attendance, EVV, daily data, behavioral data and all other documentation as indicated by the Agency and DDS.
• To be available (awake and alert) for any and all emergencies.
• Participate as a team member, communicating ideas, needs and/or concerns as necessary.
• Provide written and oral communication of both routine and unusual matters in a timely manner to the immediate supervisor.
• Responsible for following all agency policies, procedures, standards, and DDS regulatory requirements.
• Assure and maintain compliance with HIPAA rules and regulations.
• Responsible for ensuring the department is compliant with DDS QSR regulations.

ESSENTIAL FUNCTIONS:

• Responsible for daily logs when indicated.
• Follow all daily program procedures and provide assistance when necessary including, but not limited to:

o Transport clients to and from a variety of places for community inclusion

o Morning and Afternoon Transportation of participants required

o Leisure time planning and scheduling of recreation activities

o Fire drills and emergency evacuation drills

o Appropriate Social Interactions

o Follow individual specific behavior guidelines.

o Assist with mealtimes and specific dining and consistency guidelines.

o Assist with daily hygiene, toileting and oral care.

o Adaptive equipment including but not limited to wheelchairs, gait belts, leg braces, grab bars, shower chairs, Hoyer lift, dining equipment.

OTHER DUTIES:

· During inclement weather, ensure that all means of egress are clear and passable in case of emergency.

· Assist in training new staff as instructed by the Day Services Supervisor.

· Other agency projects that are deemed necessary by the Day Services Supervisor.

QUALIFICATIONS:

EDUCATION: High School Diploma or equivalent

EXPERIENCE:

· One year working with developmentally disabled adults.

· Basic computer knowledge required.

OTHER REQUIREMENTS:

· Must possess a valid driver’s license and have access to an insured vehicle.

· Must be insurable by CVS’s insurance agency

· Must have a clear criminal background history, and a clear Abuse and Neglect history through the DDS and the Office of the Inspector General (OIG).

This is a full time day position with hours of 7:30am - 3:30. Some overtime may be required.
