1915(i) Care Coordinator
Community Uplift Program
Community Uplift Program is seeking a compassionate, organized, and motivated Care Coordinator to join our team. This position works directly with participants enrolled in the 1915(i) program, helping individuals identify goals, access community resources, and build meaningful pathways toward stability and wellness.
Position Responsibilities
- Provide person-centered care coordination services to participants.
- Build positive, supportive relationships with participants and their support systems.
- Develop and maintain individualized care plans based on participant goals and needs.
- Maintain accurate and timely documentation.
- Collaborate with community partners and service providers.
- Participate in required trainings and monthly meetings.
- Connect participants to community resources, including:
- Housing supports
- Employment and educational opportunities
- Recovery support services
- Mental health and behavioral health services
- Social service programs
Qualifications
- Bachelor's degree in a human services-related field preferred; Associate's degree with relevant experience will be considered.
- Strong communication, organizational, and problem-solving skills.
- Ability to work independently while also collaborating effectively with a team.
- Basic proficiency with Microsoft Outlook, Word, and Excel.
- Reliable transportation for participant and community meetings.
- Valid driver's license, current vehicle registration, and proof of insurance.
- LGBTQ affirming and committed to serving diverse populations.
- If in recovery, must be at least 1 years sober.
Preferred Qualifications
- Experience working in human services, behavioral health, recovery support, case management, social services, or related fields preferred.
- Care Coordination Certification through the State of North Dakota.
- Knowledge of local community resources and support systems.
- Experience supporting individuals with mental health, substance use, housing, or other life challenges.

Full-time position, Monday - Friday, with flexibility for start/end times.
