The Behavioral Health Navigator is responsible for identifying, understanding and effectively connecting community residents who are experiencing mental health and substance use issues to appropriate support. This position will conduct educational activities with the broader community to increase knowledge and awareness of available mental health and substance use services and supports.

Essential Duties & Responsibilities

Complete intake and assessment with each individual.

Work closely with referred individuals to identify mental health and substance use supports.

Provide comprehensive information about appropriate referrals supports.

Conduct educational activities to the community about mental health and substance use resources.

Review assessments to identify most vulnerable participants and connect with community supports.

Develop and maintain tracking systems.

Develop and maintain systems to increase communication and understanding of mental health and substance use supports to all partnering agencies.

Develop linkages, partnerships and engagement.

Manage annual, monthly and weekly reporting expectations for the program.

Work with Individuals to access services and support for mental health or substance use.

Offer support and guidance to individuals that may need extra services.

Make referrals for mental health or substance use services for referred individuals.

Familiarize CTI staff and community partners on resources available to assist with the navigation of mental health and substance use resources.

Collaborate with community mental health and substance use providers.

Uphold the values of CTI – Equity, Integrity, Compassion.

Other duties as assigned.

Qualifications:

High School Diploma or equivalent.

Minimum of one (1) year case management, counseling or direct care mental health or substance use experience with individuals.

Strong proven leadership and decision-making abilities.

Demonstrated knowledge of the needs of low income and at-risk families.

Excellent communication skills including work within multiracial and multicultural groups.

Excellent customer service, organizational, interpersonal, oral and written communication skills.

Desire to work with low income, culturally diverse, and at-risk families and individuals.

Ability to use abstract reasoning, problem solving, planning, and analytical skills in program evaluation and improvement.

Proficient computer skills to include Windows, Microsoft Word, Excel, Internet and comfort learning new software and tracking systems.

Bilingual skills (English, Spanish, Khmer, and Portuguese) preferred.
