Recovery House Care Manager
Location: Sonoma County, CA
Schedule: Tuesday–Saturday, 10:00 AM–6:00 PM or 11:00 AM–7:00 PM

Position Overview:
The Recovery House Care Manager supports adults in a short-term post-hospitalization program. You will help residents set and achieve personal goals while fostering a positive, cooperative household environment. This role works alongside other care managers and focuses on evidence-based approaches to care and rehabilitation.
Key Responsibilities:
Client Services:
- Conduct outreach, intake, and needs assessments for potential residents.
- Collaborate with residents to develop and implement individualized Care Plans.
- Provide ongoing case management, including benefits assistance, documentation support, employment guidance, housing navigation, healthcare referrals, and community connections.
- Apply evidence-based practices, including Motivational Interviewing, to promote independence and social responsibility.
- Provide crisis intervention and prevention as needed.
Resident Support:
- Facilitate educational classes, group sessions, and recreational activities.
- Support residents in maintaining household responsibilities and program compliance.
- Transport residents using agency vehicles when necessary.
- Assist with rent collection and facility upkeep, ensuring a safe, welcoming, home-like environment.
Program & Compliance:
- Participate in staff meetings, supervision, trainings, and program planning.
- Follow confidentiality guidelines, agency policies, and state/federal regulations.
- Ensure program compliance with contractual and licensing requirements.
Recordkeeping:
- Maintain accurate and timely documentation of resident interactions, services provided, and incident reports.
Minimum Qualifications:
- High school diploma or equivalent.
- At least one year of experience working with adults with mental health challenges (or equivalent).
- Ability to meet licensing and background check requirements.
- Reliable transportation, valid CA driver’s license, and insurability under agency auto policy.
- Strong observation, listening, and communication skills.
- Proficiency in Microsoft Word, Excel, and standard computer programs.
Preferred Qualifications:
- Some college coursework in social services or related field (or equivalent work experience).
- Knowledge of social rehabilitation best practices.
- Familiarity with Sonoma County Behavioral Health (SCBH) services and resources.
- Experience with team-based treatment and program planning.
- Community Health Worker experience or training.
Physical Requirements:
- Lift/carry up to 20 lbs occasionally.
- Ability to climb stairs and walk with clients up to 2 miles.
- Occasional reaching and ladder use.
