Job Summary
We are seeking a dynamic and relationship-driven Provider Relations & Outreach Specialist to join our growing healthcare organization. In this role, you will be responsible for building and maintaining strong relationships with service coordinators, case managers, and healthcare professionals within the Intellectual and Developmental Disabilities (IDD) and Home & Community-Based Services (HCBS) space.
As a key driver of growth, you will promote our services, generate client referrals, and position our organization as a trusted, responsive, and high-quality provider. Your ability to communicate effectively, follow up consistently, and build long-term professional relationships will directly contribute to expanding our impact and improving outcomes for individuals receiving care.

Responsibilities
- Develop and execute strategic outreach plans to build relationships with service coordinators, case managers, hospitals, and community partners
- Conduct in-person and virtual outreach, including meetings, calls, and follow-ups to maintain strong referral pipelines
- Serve as the primary point of contact for referral sources, ensuring consistent communication and responsiveness
- Educate referral partners on company services, service offerings, and program capabilities
- Track all outreach activities, relationships, and follow-ups using internal CRM systems
- Identify new referral opportunities and proactively engage potential partners
- Maintain consistent follow-up with contacts to stay top-of-mind for client placements
- Collaborate with internal teams to ensure smooth onboarding and service delivery for referred clients
- Represent the organization at community events, networking functions, and healthcare-related meetings
- Monitor outreach performance and provide insights on referral trends and growth opportunities
Experience
- Proven experience in healthcare outreach, sales, account management, or relationship-based roles (IDD, HCBS, or social services experience preferred)
- Strong ability to build trust and develop long-term professional relationships
- Excellent communication and interpersonal skills, with the ability to engage diverse stakeholders
- Demonstrated ability to manage multiple relationships and follow-ups in an organized and structured manner
- Self-motivated with a results-driven mindset and strong follow-through
- Experience working with service coordinators, case managers, or healthcare professionals is highly preferred
- Ability to work independently while contributing to a collaborative team environment
- Strong organizational and time management skills
- Proficiency in Microsoft Office (Word, Excel, Outlook); experience with CRM systems is a plus
