Community Skilled Health Care Centre is seeking skilled, compassionate, and dependable Licensed Practical Nurses (LPNs) to join our growing night shift team. Our mission is to provide patient-centered care while fostering a supportive, respectful, and team-driven environment.
Licensed Practical Nurse (LPN) Requirements:Active Ohio LPN license
Long-term care experience preferred, but not required
Strong clinical and communication skills
Ability to work independently on night shift
Administer medications and treatments as prescribed
Monitor and document residents’ conditions, including vital signs and changes in status
Assist in developing and updating individualized care plans
Provide direct nursing care including wound care and daily living support
Collaborate with RNs, physicians, and interdisciplinary team members
Ensure compliance with all facility and regulatory standards
Respond to emergencies and provide appropriate care
Supervise and support nursing assistants (STNAs/CNAs)
