What are the responsibilities and job description for the Direct Support Professional position at Community Services of Virginia, Inc.?
DUTIES AND RESPONSIBILITIES (include but are not limited too/brief summary below) :
- In Home Staff will provide one-on-one direct care having responsibility for guidance and supervision of the individuals within the home. Meet with individual on scheduled days as agreed with individuals’ parents/legal guardian and individual.
- Oversee and ensuring the needs of individuals are properly met i.e., physical care; development of acceptable habits and attitudes; training skills’ community integration and helping to meet the goal and objectives of any service plan.
- Accompany individuals to and from training and community activities as assigned; provide direct supervision and guidance to ensure safety and welfare of individuals’.
- Adheres to already pre-established procedures to report any changes and/or concerns to appropriate persons; observes individuals daily for behavioral changes i.e. physical injuries/conditions and cleanliness.
- Completes data collection according to Individuals Service Plan (ISP); plans and implements special activities; completes special projects and other duties as assigned.
- Responsible for possible intervention; the process of carrying out a plan of action to resolve a human occurrences
