What are the responsibilities and job description for the Health Aide position at Community Service Board Of East Cen?
- PURPOSE AND SCOPE
Provide basic patient care and work with individuals to develop daily living skills, community resource linkage and coordination of care during admission and at discharge. Work collaboratively with the medical staff to monitor individual’s medical needs (ie; medication complaints, vital signs, etc.).
- DUTIES AND RESPONSIBILITIES
- Orients individual served to the unit and assists with completing required admission paperwork.
- Assists in administration of medications and treatments, takes vital signs, and/or collects urine specimen for specified lab test, as directed by a physician or nurse.
- Assist individual served with activities of daily living such as hygiene and laundry.
- Conducts group activities for education, recreation and socialization.
- Ensures safety and security of individual served.
- Observes and documents patient data, behavior and activity.
- Prepares individual served for treatment or examination.
- Collaborate with the medical team to ensure care of all individual served.
- Prepare all items needs for the following shift staff.
- Assist individual served with daily living skills/activities as ordered by the provider or documented in the individual served IRP.
- CSU Health Aide positions do not drive any vehicles.

JOB REQUIREMENTS:

- Ability to take individual served vital signs.
- Ability to chart individual served behavior.
- Ability to provide to the individual served simple nursing care, personal care and hygiene.
- Ability to explain procedures and policies to individual served.
- Ability to perform established treatment and programmatic activities.
- Ability to chart individual served behavior.
- Follow all SBHS policies and procedures.
- Present to assigned shift/schedule on time.
- Present in attire following the CSU dress code.
- Complete all required annual training courses.
