What are the responsibilities and job description for the Life Coach position at COMMUNITY SENIOR SERVICES INC?
Summary:
The Life Coach position is a full-time position. The Life Coach is responsible for providing guidance and assistance to Participants in the Florida Self-Directed Care Program regarding their long-term needs and goals, and the funds available to the Participant for their mental health treatment and social services. The Life Coach is also responsible for determining a candidate’s program eligibility. The Life Coach is expected to develop and implement a Life Action Plan with each Participant based upon their identified needs and goals ensuring that the objectives in the Life Plan are tied to recovery and are attainable. The Life Coach will provide quality services and referrals in order to assist Participants with attaining empowerment, self-sufficiency, independence and recovery. The Life Coach must possess cultural sensitivity and be aware of the critical factor that culture can play in the recovery of mental illness. The Life Coach must be dedicated to their responsibilities and passionate about helping diverse persons in need of their assistance as they participate in our programs.
Knowledge:
The Life Coach is expected to exhibit knowledge in WRAP (Wellness Recovery Action Planning), Life Analysis Planning, PHQ-9, and Recovery Planning.
Qualifications/Requirements:
· Requires a Bachelor’s degree in behavioral health
· Preferred three (3) years of professional experience in mental health, social work, counseling, or a behavioral health field working with individuals with serious mental illnesses.
· Valid Driver's License and auto insurance
· Reliable transportation to travel throughout the community
· Competence and sensitivity in providing services to persons of diverse cultural backgrounds.
· Understand the following concepts of empowerment, cultural sensitivity and person-centered approach therapy and the role that they play in providing services to persons with mental illness of diverse cultural backgrounds.
· Possess organizational skills and ability to develop a Life Action Plan that is easy to follow, measurable and attainable.
· Ability to observe and define Participant’s needs before they become problems.
· Have experience providing services in a Participant’s home and other community settings.
· Pursuant to Chapter 435, F.S. Mental Health personnel must pass a Level II background check and drug screening.
· The ability to work independently.
· Possess strong written and oral communication skills.
· Experience and knowledge of Microsoft Office applications.
· Experience with various computer software programs or the ability to quickly grasp the concepts of new computer software and applications.
· Must be able to complete a course related to the skills, practice, and principles of recovery planning and pass a related examination prior to employment with the program.
Specific Duties:
· Ensure that candidates meet all eligibility requirements and that participation in the program is clinically appropriate.
· Within fifteen (15) days of enrollment, provide education and training, including budgeting training, and orientation to new Participants.
· Daily notes are logged within 48 hours of assignment.
· Assist each Participant with action plan and budget management responsibilities to address all priority areas of the Participant to promote their Life Action plan within ten (10) days of new Participant orientation.
· Provide training and technical assistance regarding developing a purchasing plan, selecting, hiring, and managing service providers, and monitoring quality of care.
· Establish an individually tailored budget in accordance with the participant’s needs and personal preferences. Provide training on managing funds available for their mental health treatment and support services.
· Assist and educate the Participant with securing additional state and federal benefits (i.e., Florida Medicaid, Social Security Disability Insurance and Supplementary Security Income).
· Maintain precise and accurate records reflecting the treatment sources used and the recovery and support services offered to each Participant.
· Assist and participate in the discharge planning of individuals enrolled in the Florida SDC program.
· Participate in Department of Children & Families sponsored training events in the areas of recovery and Self-Directed Care when such training is made available in addition to any related educational courses that DCF deems pertinent to keeping the Life Coaches current with new trends within their area of specialty.
Working Conditions/Physical Requirements:
· Business hours are Monday through Friday, 8:30 a.m.-5:00 p.m.
· Due to circumstances that may arise, overtime may be required at times. In all cases, overtime must be pre-approved by your supervisor.
· Must be able to remain in a stationary position 50% of the time.
· Requires sitting at a computer for up to eight hours a day.
· The person in this position will occasionally (takes up to one-third of their time) to move about inside the office to access file cabinets, office machinery, etc.
· Constantly operates a computer other office productivity machinery such as a calculator, copy machine, and computer printer.
· The person in this position frequently communicates with clients who have inquiries or questions. Must be able to exchange accurate information in these situations.
· Frequently in client home settings.
